All Florida medical marijuana patients and their caregivers must fill out the appropriate applications to be approved for a medical marijuana card. The application can only be filled out and submitted after the patient has been put into the medical marijuana registry by a qualified ordering physician. Once the physician has put the patient into the state registry, the Office of Medical Marijuana Use will send an email notifying the patient they are accepted into the registry. Upon obtaining this email, the patient will be allowed to register their online profile with the Florida medical marijuana registry. Once the patient is logged in they can complete a simple online application and pay directly through the website using a credit card. The system will automatically pull your image for the medical marijuana card if you have a image on file with the DMV or Passport office.
An application for a Florida medical marijuana card must be completed for all patients and their caregivers if they have one. Patients that have been assigned a caregiver will not be allowed to purchase medical cannabis products from a Florida dispensary until their caregiver completes an online application. If for some reason you’re unable to fill out the online application please follow the steps below and print the appropriate paperwork to complete and mail into the state with your $75 processing fee.
Florida Medical Cannabis Patients
Along with the application, the patient must submit
- A copy of your Florida driver license, Florida identification card, or other proof of residency.
- A $75 check or money order (application fee) made out to Florida Department of Health.
- A passport-style, color photograph (2×2 inches in size) taken within the 90 days immediately preceding application. (Walgreens, CVS and The UPS Store all do passport photos)
If you are applying for a child under the age of 18 or as a patient’s caregiver, you must also include:
A Compassionate Use Registry Identification Card Legal Representative Application.
A copy of the parent’s or designated legal representative’s proof of residency.

Once your Identification Card application has been approved, you will receive a temporary identification via email to be used to fill your medical cannabis order until your physical medical marijuana card arrives.

Florida Medical Marijuana Card Renewals:
To maintain an active Medical Marijuana Use Registry identification card, all patients and caregivers must complete an annual renewal application. The annual renewal application needs to be submitted with any required documentation forty five days prior to the expiration date of the card. All mmj cards expire one year from the date of approval. Renewals for patient and caregiver cards may be submitted online or with a paper application. When submitting a renewal application by paper, please mark the check box at the top of page 3 that says Renewal Application.

To change, replace or surrender your Medical Marijuana Use Registry Identification Card, fill out the Change, Replacement or Surrender Request form and mail your $15 payment to the address below.
